Digital Assistance for Bluebell Lane Medical Practice Patients

As you will be aware, Bluebell Lane Medical Practice merged with Cornerways Medical Centre in April 2024.

As a result of this merger, patients utilising the NHS App, who have migrated from Bluebell Lane Medical Practice are required to delete their existing application and re-instate it with the new practice details.

The process is relatively straight forward, but we understand that some patients may be having difficulty to carry out this process.

There are three key pieces of information that are required:

  • A personalised Linkage Key (could be called a Passphrase)
  • Your practice ODS Code (could be called an Organisation Code or Practice ID)
  • Your Account ID

These three key elements are available by simply completing the following form:

This form is to request the practice to provide a patient Linkage Key, ODS Code, and Account ID.

When you have this information, you can use the video below to help you carry out the process of deleting your previous account and re-registering using your new practice details.

Please note that deleting your previous account and re-installing to your new Cornerways practice, will not lead to any of your personal or medical information being lost or amended.
